Reading the patient header
Demographics, last visit, current conditions, and the share button — what every field means.

The patient header sits at the top of every chart page. It is the fastest way to confirm you are looking at the right person and to jump into common actions.
What is shown
- Name and date of birth, with age computed at the time of the visit.
- Sex, MRN, email, and phone if filled in.
- Last visit date and the clinician who signed it.
- Current active conditions, with a count badge.
- The Share button — opens the share-link panel.
What is not shown
- Encounter notes — click into the Visits tab to read those.
- Photos — they are stored against visits, not the chart directly.
- Audit history — see Admin → Platform for the audit log.
