The diagnosis review flow

The diagnosis review panel is where you turn a Lens suggestion into a tracked condition. It walks through four steps: pick the differential, attach an ICD-11 code, write the assessment, and create the condition.

Step 1 — Choose a differential

Lens produces a ranked list. Click the differential that best matches your clinical read. You can also click “None of the above” to skip Lens entirely.

Step 2 — Pick an ICD-11 code

The code picker calls lib/icd10.ts to suggest the top matches. You can search by code or description; results are ranked by exact code, prefix, and keyword match. Pick the most specific code that applies.

Step 3 — Write the assessment

Free-text field. Include the reasoning behind your differential, anything you considered and ruled out, and the plan. This text becomes the encounter note if you do not write a separate one.

Step 4 — Create the condition

1Confirm the body location and side from the anatomy picker.
2Click Save. The condition appears on the patient’s Conditions tab.
3Future visits attach to the condition automatically when the same body location is photographed.

Editing a condition after it is created does not change the original ICD-11 code; it adds a new code entry so the history is preserved.
