Writing an encounter note

Create an encounter from a visit, write the note, sign it, and add an addendum if you need to amend it.

An encounter is the clinical narrative for one visit. One visit can have one signed encounter note; if you need to amend it, you append a signed addendum rather than editing the original.

Creating an encounter

  1. 1Open the patient’s chart and click the Visits tab.
  2. 2Click “New encounter”.
  3. 3Choose the visit this encounter belongs to, or create both at once.
  4. 4Write the note in the editor. Markdown is supported.

Signing

When the note is ready, click Sign. Signing writes your name, the timestamp, and the note text into encounters and locks the row from edits. After signing, the encounter appears on the visit and in the patient’s overview.

Signing is permanent. If you need to change a signed encounter, append an addendum rather than trying to edit it.

Adding an addendum

  1. 1Open the signed encounter.
  2. 2Click “Add addendum” at the bottom of the note.
  3. 3Write the additional information.
  4. 4Sign the addendum. It appears below the original note, with its own signature and timestamp.
