Attaching ICD-11 codes to a condition

Search the ICD-11 catalog, pick the most specific code, and understand how codes appear in the patient’s record.

SkinMD ships with a curated ICD-11 MMS dermatology catalog stored in the icd11_codes table. Codes are searchable by code, description, category, and keyword, and the picker ranks results by clinical relevance.

Searching for a code

  1. 1Open the diagnosis review panel.
  2. 2Type at least two characters into the ICD-11 search.
  3. 3Results are ranked: exact code match → code prefix → keyword match → description contains.
  4. 4Click a row to attach it.

How codes are stored

  • Codes are append-only — adding a new code does not remove the previous one.
  • Each code entry records the clinician, the timestamp, and the source (Lens suggestion or manual).
  • The condition timeline shows the full code history, newest first.

If you cannot find a code, it may be inactive in our catalog. Use the “Request code” link in the picker — we add new codes within one business day.
