Your first patient chart

Create a patient, open the chart, and understand the tabs (visits, encounters, assessments, conditions, share).

A patient chart is the container for everything SkinMD knows about one person: visits, encounters, AI-assisted assessments, conditions, ICD-11 codes, and the share link. Charts are scoped to the clinician who created them — another clinician in the same clinic sees only the patients they have been granted access to.

Creating a patient

  1. 1Open the dashboard and click Patients in the side navigation.
  2. 2Click “New patient” in the top-right of the list.
  3. 3Enter the patient’s full legal name and date of birth. Optional fields can be filled later.
  4. 4Click Create. You land on the new chart with no visits yet.

Reading the tabs

OverviewDemographics, last visit, current conditions, share link button.
VisitsEvery documented encounter for this patient, newest first.
AssessmentsAI-assisted differential assessments produced for this patient.
ConditionsDiagnoses tracked over time, with ICD-11 codes attached.
ShareIssue and revoke share links for this patient.

Tabs appear only when there is data for them. A new patient will show just Overview until you create a visit.
