Patient records are created from the dashboard Patients page. Required fields are full legal name and date of birth; everything else can be filled in later.
Required vs. optional fields
| Field | Required? | Notes |
|---|---|---|
| Full legal name | Yes | Used in the share-link summary and visit PDFs. |
| Date of birth | Yes | Used for age-based reminders. |
| Sex | Yes | Used for screening reminders. |
| No | Only used if you send a share link to the patient. | |
| Phone | No | Free-form; used by the clinic, not by SkinMD. |
| Notes | No | Free-form; visible to clinicians in your clinic. |
Editing a patient
Open the patient from the Patients page, then click Edit in the patient header. Edits are saved through the updatePatientAction server action. The previous values are not preserved — if you need to correct an error, type the new value and save.
SkinMD does not currently store patient name change history. If you need a paper trail, note the correction in the patient’s next encounter.
