Early detection of melanoma is critical for patient outcomes. The ABCDE criteria provide a systematic approach to evaluating pigmented lesions and identifying potential melanomas.

The ABCDE Framework

The ABCDE criteria represent five key characteristics that help distinguish melanoma from benign nevi:

  • A - Asymmetry: One half doesn't match the other
  • B - Border irregularity: Edges are ragged, notched, or blurred
  • C - Color variation: Multiple colors or uneven distribution
  • D - Diameter: Larger than 6mm (though melanomas can be smaller)
  • E - Evolution: Changing in size, shape, or color

Clinical Application

When examining patients, it's essential to document baseline images and monitor for changes over time. Digital dermoscopy allows for precise tracking of lesions and early identification of concerning changes.

The Ugly Duckling Sign

Beyond ABCDE, the "ugly duckling" sign identifies lesions that look different from a patient's other moles. This is particularly useful for patients with multiple nevi, where individual lesions may not meet ABCDE criteria but the overall pattern suggests concern.

When to Biopsy

Any lesion demonstrating ABCDE features or the ugly duckling sign warrants consideration for biopsy. When in doubt, it's better to biopsy and have pathology confirm a benign lesion than to miss an early melanoma.
